Student Aid Application
Print the Student Aid Application and complete in full.
Applications are processed by an independent firm, Private
School Aid Service (PSAS). Application packets should be mailed to PSAS
and include the following. Incomplete applications cannot be processed and
will delay notification of awards.

Signed and dated
application form.

Copies of required tax
returns and schedules.

Copies of income forms such
as W-2s, 1099s, etc.

Copies of documentation for
all 2011 all non-taxable income.

Copies of court decrees or
agreements outlining child support or alimony to be paid or received, custody
and tuition responsibility, if applicable.

Check or money order
payable to Private School Aid Service for the non-refundable application fee of
$25.
