Transcript Request

To request a transcript from Central Catholic:

Mail your request to:
     Central Catholic HS
     Attn:  Guidance Office
     2410 S. 9th Street
     Lafayette, IN  47909

Call in your request to (765) 471-8362.

Email your request.

Please include the following:  Last Name, First Name, Middle Initial, Maiden Name, Date of Birth, Year of Graduation, Current Phone Number, School Attended, and where transcripts are to be sent.  Please allow one week for processing.

Please Note:  If your transcript is sent to you for a school, do not open it.  It will not be an official transcript if it has been opened.
